DISKERY RELEASE ENTRY APPLICATION

(Items marked in RED MUST be answered, all others may be left blank if you don't know or it doesn't apply to you).

After you have filled out this form, you must obtain a sales Agreement form. Your application will not be accepted until it is correctly filled out and the Agreement has been signed and mailed to us. You will also be required to send sample materials (sample of recording as well as any pictures, biography and other marketing materials). All new recordings must first be approved before they are available for sale. The Distribution Papers page will be presented when you complete this form.

This form is not required, you can simply submit the appropriate Distribution Papers, and allow us to input your information manually. We PREFER if you use this form, however, as it will speed up your input process. If you are not supplying marketing materials of band biographical information then you must complete this page.

PRICING

In this section you must enter the pricing for this recording.

The WHOLESALE price is the price you will be paid when your title sells. You must also select which currency you are basing the wholesale price. You can only select between Canadian and US Dollars, therefore if your home currency is different from these you will have to calculate your price into one of these two currencies. A

is provided to assist you. A good "rule of thumb" to remember is to make your price reasonable, you want to cover your costs and make money but the higher the price you charge us, the higher the price will be to the customer. Try to sell the music at the lowest price possible. In General, wholesale prices around $5-10 are the normal. The price that will appear to the customer on your release page will be this price plus $2.

Your Wholesale Price: $

Before you quote a price, it is a good idea to know what it is in your home currency. To find out more
