After you have filled out this form, you must obtain a sales Agreement form. Your application will not be accepted until it is correctly filled out and the Agreement has been signed and mailed to us. You will also be required to send sample materials (sample of recording as well as any pictures, biography and other marketing materials). All new recordings must first be approved before they are available for sale. The Distribution Papers page will be presented when you complete this form.

The WHOLESALE price is the price you will be paid when your title sells. You must also select which currency you are basing the wholesale price.
